Colonial Terrace Animal Hospital welcomes new
patients and clients. As a practice dedicated to wellness, our mission
is to see pets
as often as necessary to maintain good health and prevent illness.
However, we are always happy to see pets presented for illness,
regardless of whether the owner or pet has been here previously.
If you are new to Colonial Terrace Animal Hospital, we may ask for
previous medical records that are pertinent to your pet. Our receptionists
are always happy to contact veterinary hospitals where the pet was
seen previously to obtain necessary vaccination or medical history.
Trying to fill out paperwork while hanging on to your pet can be
a challenge. That is why we’ve made it possible for you
to download and print the check-in form below to fill out at home
and
bring into your first visit or e-mail to us at dvmctah@aol.com.

Don’t forget if you are bringing
in a new pet, or any pet that hasn’t been examined
by a veterinarian in over one year; bring along a stool
sample from your pet. (This
allows
us to check
for
intestinal parasites, which can potentially be transmitted
to humans.)
APPOINTMENTS
To allow adequate time for our patients, we schedule all exams, vaccinations,
and surgical procedures by appointment. Of course, emergencies
are inevitable and sometimes we fall behind schedule because of
them. Please understand this and realize we make every effort to
stay on schedule when possible. We appreciate a telephone call
if a scheduled appointment needs to be changed.
METHODS
OF PAYMENT
We make every effort to provide your pet with the most up-to-date
medical and surgical care. Our prices reflect this and are considered
fair and reasonable. You can obtain a written estimate for any
treatment recommended by our doctors, and you will receive an itemized
receipt for any services provided.
We accept cash, personal checks, Visa, MasterCard, Discover and American
Express. Payment in full is expected at the time of services unless
special arrangements have been made in advance with our practice
manager.
